Addressing Client Needs with Social Determinants of Health Scale (ACN:SDH) to be used by school counselors

Hello School Counselor! 

We are currently conducting a study for school counselors that focuses on adapting and evaluating the Addressing Client Needs with Social Determinants of Health Scale (ACN:SDH) to be used by school counselors. Because you are a school counselor, your opinions are important to this study. 

Participation in this study will involve the completion of a 15-20 minute survey. There are no known or anticipated risks to your participation in this study. The questions asked are general. You may decline answering any questions you feel you do not wish to answer. You will receive not direct benefit from participating in this study. Participation in this study is voluntary.  

All information you provide will be considered confidential and grouped with responses from other participants. You will not be identified by name in any report or publication resulting from this study. You may withdraw from the study at any time without penalty or loss of benefits to which you are otherwise entitled.   

 If you choose to participate in the survey, you have the opportunity to enter a drawing to receive one of ten $25 Amazon e-gift cards. You will be directed to a separate online page where you can provide your name and email in order to receive the gift card by email. Your survey results will remain confidential and separate from your gift card entry.
